Personalised Learning Plans (PLPs)

A Personalised Learning Plan (PLP) is a document that is central to the planning, resourcing, modelling and review of pupil’s individual outcomes that link to their Education, Health & Care (EHC) Plan. 

A PLP acknowledges that the broad and rich outcomes outlined in a pupil’s EHC are required to be broken down into manageable, smaller SMART outcomes that are achievable over a term.

PLPs are live documents that are engaged with regularly by all members of the class team and should capture the interventions and provision that each pupil receives in order to make good progress against their outcomes. PLPs link closely to the pupil’s learning map which is a detailed overview of the young person’s wants, needs and motivators as well as how parents/carers can support teaching and learning at home.

 

Learning Maps

Learning Maps are a document that give staff an overview of the pupil they are working with that includes;

  • the most up-to-date EHC Plan outcomes for the pupil
  • key information (e.g. name, D.O.B., year group)
  • what is important for the pupil
  • how the pupil can be best supported in school
  • how the pupil can be supported at home
  • key assessment data 

The Learning Map for each child is created by the SENDCo & Class team following on from an annual review and it is then circulated to all staff that work with the child. Parents are also given a copy of this document for their records. ​
